Texas Long Term Services and Supports (LTSS)

Community First Choice  is a state plan option separate from the waiver.

CFC services consist of the following:
CFC personal assistance services - Habilitation (PAS/HAB); and
 CFC emergency response services (ERS)
CFC PAS/HAB allows an individual to reside successfully in a community setting by:
 Assisting an individual in performing activities of daily living (ADLs) and instrumental activities of daily living (IADLs);
 Assisting the individual in acquiring, retaining, and improving self-help, socialization, and daily living skills; and
Assisting with and training the individual on ADLs and IADLs.

Respite is the short-term relief of the unpaid primary caregiver of an individual who lives in their family home. Respite is provided intermittently when the primary caregiver is temporarily unavailable to provide supports due to non-routine circumstances. This service assist in providing an individual with personal assistance with activities of daily living. This includes habilitation activities and inclusion in community activities, use of natural supports and typical community services available to all people, social interaction and participation in leisure activities, and the development of socially valued behaviors, daily living, and functional living skills.

Day Habilitation assist in providing individuals with assistance in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to live successfully in home and community-based settings. Day Habilitation provides for individualized activities in environments designed to promote the development of skills and behaviors supportive of greater independence and personal choice and consistent with achieving the outcomes identified in the individual’s service plan.

Nursing services are those services that are within the scope of the Texas Nurse Practice Act and are provided by a registered nurse or licensed vocational nurse under the supervision of a registered nurse licensed to practice in the state of Texas. Nursing services provided in the waiver cover ongoing chronic conditions such as medication administration and supervising delegated tasks. This broadens the scope of these services beyond extended State Plan services. Dental services are those services provided by a dentist to preserve teeth and meet the medical need of the member and are provided under the waiver when no other financial resource for such services is available or when other available resources have been used.

Employment Assistance is provided to an individual to help the individual locate paid employment in the community. EA includes: identifying an individual's employment preferences, job skills,  requirements for a work setting and work conditions; locating prospective employers offering employment compatible with an individual's identified preferences, skills, and requirements, and contacting a prospective employer on behalf of an individual and negotiating the individual's employment. Supported Employment is provided to assist the individual served with sustaining competitive employment in the community. Individuals receiving supported employment earn at least minimum wage unless self-employed.

Includes the Community Supports Services transportation necessary for the individual served to participate in community activities or assistance in securing such transportation. CSS transportation is an exclusive service unique to the LTSS waiver.
